New Employees to the Groden Network

Welcome! Below we have provided the forms that you will need to begin your employment at the Groden Network, (as well as some information about your benefits). Please print and complete the required forms below and bring them to Human Resources prior to your first day of work. If you have any questions, please feel free to call Human Resources at 274-6310, ext. 1233, or ext. 1297.

Section I – Required for All Employees

Please bring these completed forms to Human Resources prior to your first day of work:
These forms are a High Priority and they must be completed prior to completion of remaining paperwork for processing.

Other Required Documents

Section II. Additional Requirements for Staff Working in Groden Center Programs, Kingston Hill Academy, HTS & PASS Programs

Please bring these completed forms to Human Resources prior to your first day of work.

  • Documentation of Immunizations
    • MMR (Measles, Mumps, Rubella) Considered immune if born prior to 1956. If you can not find documentation of this then you need to receive a Titer Test (Blood Test)
    • TB test (completed within the last 6 months)
    • Tetanus (completed within the last 10 years)
    • Driver's Medical Certificate (Must be completed by Groden Day and Groden Residential Staff. An appointment must be made for this service).

* If you do not have documentation of immunizations, you may go to Corporate Care at Our Lady of Fatima Hospital to receive immunizations. The Groden Network will pay the cost of services.

* If services are performed by a personal physician, the Groden Network will not pay the cost.

Section III. Additional Requirements for Staff Working in Cove Center Programs

Please bring these completed forms to Human Resources prior to your first day of work.

Section IV. Additional Requirements for Staff Working in Halcyon Program (Behavioral Associates of Massachusetts)

Please bring these completed forms to Human Resources prior to your first day of work.

Note: Massachusetts Residents can go to RI Attorney General’s Office or RI State Police Barracks.
