Supportive Living Arrangement Coordinator

Be responsible for coordinating services for adults participating in Cove’s Supported Living Arrangement (SLA), including the provision of quality person-centered case management. The Coordinator is also responsible for providing effective training, evaluation, and monitoring of SLA Contractors as well as ensuring Respite services are managed successfully. The Coordinator will actively participate in referral evaluation, record review, screening, interviews, and the matching process. Prior to placement, the Coordinator will determine the behavioral and clinical needs of person seeking services, identify the family’s needs, and conduct an assessment of the Contractor’s home to identify any safety issues or other potential risks. The Coordinator is expected to conduct in-home, face-to-face visits with the person receiving services. In-home visits must occur minimally one time per month but with increased frequency as determined by the needs of the person receiving services or the Contractor. In-home visits should include the opportunity to be an effective communicator with the SLA, discuss concerns, observe the Contractor’s interaction with the person receiving services, demonstrate strategies to proactively resolve issues, and provide effective feedback. Must attend SLA meetings. Dissemination and participation in Network activities and events.

Qualifications:

Must have a bachelor’s degree in Psychology or related field and experience working with adults with developmental disabilities. The qualified candidate will have prior supervisory experience, the ability to work collaboratively with others, and demonstrated knowledge of quality person-centered community living options. Must have strong computer skills and a working knowledge of BHDDH.

Labor Status: Part-Time

Hours: 20-25 hours per week
