New Employee Forms

Welcome! Below we have provided the forms that you will need to begin your employment at the Groden Network, (as well as some information about your benefits). Please print and complete the required forms below and bring them to Human Resources prior to your first day of work. If you have any questions, please feel free to call Human Resources.


Section I – Required for All Employees

Please bring these completed forms to Human Resources prior to your first day of employment:


Section II. Additional Requirements for Staff Working in Groden Center Programs, Kingston Hill Academy, HTS & PASS Programs

Please bring these completed forms to Human Resources prior to your first day of employment:

  • Documentation of Immunizations – If you do not have documentation of immunizations, services are provided at Corporate Care at Our Lady of Fatima Hospital to receive immunizations. The Groden Network will pay the cost of services.
    • MMR (Measles, Mumps, Rubella) – You are considered immune if born prior to 1956. If you can not find documentation of this then you need to receive a Titer Test (Blood Test)
    • Tuberculosis (TB Test) – (must be completed within the last 6 months)
    • Tetanus- (must be completed within the last 10 years)View
    • Driver’s Medical Certificate (To be completed by Groden Day, Pre-School and Groden Residential Staff only. (Services are only provided at Concentra Health Centers)
      • You will need to contact Human Resources (see contact information at top of page) to obtain Authorization of Services Form prior to having your driver’s medical exam. No appointment is needed for this service.

      • If the above services are performed by a personal physician, The Groden Network will not reimburse you for the cost.
  • DCYF Clearance Request (DOC) (Please complete this form and return to Human Resources with a $10.00 cashier check or money order prior to completing all other documentation. Please return this form as soon as possible. Human Resources will mail this form to DCYF.)
  • Criminal Background Check (BCI) – Must be completed at the Attorney Generals Office.
  • Fingerprint Affidavit (PDF) – Must be completed at your local Police Department.
    If you are a Massachusetts resident, you must complete your Fingerprinting at one of the locations of the Rhode Island State Police barracks. (We do not accept fingerprint cards)

Section III. Additional Requirements for Staff Working in Cove Center Programs (Section II does not apply)

Section IV. Additional Requirements for staff working with Behavioral of Associates of Massachusetts (Section II & III do not apply)

Please bring these completed forms to Human Resources prior to your first day of employment:

  • CORI Request Form (PDF) – Required for all Behavioral Associates of Massachusetts staff (Please complete this form and return to Human Resources prior to completing all other documentation. Please return this form as soon as possible to Human Resources so this form can be processed.)
  • Criminal Background Check (BCI) from Attorney General’s Office (Must be completed if you are a current or previous resident of Rhode Island)

Note: Massachusetts Residents can go to RI Attorney General’s Office or RI State Police Barracks.
